Policy of Credentials

Celebration Life Ministries offers credentials to those who have demonstrated a heart for ministry. The potential individual will follow biblical examples of godly character and serve in the local church.

 

The individual will need the following:

 

1. A completed personal information form

2. A written copy of your personal testimony of salvation and baptism of Holy Spirit

3. Copy of educational certificate

4. A completed form of financial disclosure

5. A signed acknowledgment of having read and agreed to Celebration Life Ministries

6. A completed form of our doctrinal questionnaire

We ask that you fill out all portions of this application for yourself. We recommend you type out answers in a separate computer document, then copy and paste them into the form once you have them all complete. You will not be able to save your progress in the online form.
